As a patient with endometriosis you have a few options you can discuss with your doctor. 1. medication: continuous progestins or combination oral contraceptives until the endometriomas shrink; antiestrogens until the endometriomas shrink. 2. surgery: conservative surgery can be performed to lyse adhesions and enhance fertility. Your physician / gynecologist will be better able to advise you what is optimal for your situation (endometriosis). Your irregular periods combined with your sore / hard nipples points towards an anovulatory menstrual cycle which requires a complete workup by a gynecologist. So in short you have a complex situation which is treatable if addressed by a gynecologist. Hope it helps some.
