It’s hard to say. The baby could have gotten the gene for down syndrome from the sister or cousins partner. You should look and see if your direct relatives such as your mom, dad, grandma, and grandpa had any direct blood relatives with down syndrome. Also since down syndrome is a recessive trait it would take both you and your partner to carry the trait for down syndrome in order for it to be present and become dominant in your child.

Yes they are. But it all has to do with the gene. Because usually that gene could be hidden in your dna and can go to the next one. But I doubt if your first child were to have that, then you can continue to have children.
